Unilateral triangular cataract in an old man

Clinical image description

A 64-year-old man complaint about low vision of the right eye long time ago, presented to change his glasses. The best corrected visual acuity was 5/10 OD, and 10/10 OS. On examination, we found a central triangle-shaped cataract in his right eye that is better shown by retro illumination (Figure 1). The rest of the exam is unremarkable.

The interrogation eliminated any notion of ocular trauma, drug intake, inflammatory condition, or family history of cataract.

Prior research has linked prenatal methamphetamine exposure [1] or genetic mutation [2] to bilateral triangular cataracts. The patient’s monocular appearance raises the possibility of an unidentified local cause.
